When the same problem keeps resurfacing, the cause is usually structural. The quoting process depends on one person's memory. Two departments own halves of the same outcome. The report that would have caught it early does not exist, or nobody reads it.

There are 7 conditions that set the ceiling on what any team can produce: structure, process, resources, information, innovation, training and engagement. Leadership owns all 7. Most recurring operational problems trace back to one or two of them, and the SPRIITE survey is how we find out which.

From there the work is practical. Org design and reporting lines, the processes that keep failing, the measurement that would have surfaced the problem a quarter earlier, and the accountability that makes a fix hold after the attention moves on. Behind that is over two decades as an operator and executive, including full P&L ownership and the accountability that comes with it.

The team should be able to recognize the same pattern next time without me in the room.
